Annual Self-Review
Hi,

I have had to create my own annual review for the past eight years. I never had an example to go by, and I am wondering if anyone would like to share theirs so I can get an idea if my format is correct. I always start out with a summary of the past year, achievements in bullet points, a "looking ahead" section, and then a conclusion. I would like to see the meat and potatoes of one to get an idea of how I could improve mine.

Thanks a million!

Re: Annual Self-Review
We also have to do a self review annually but ours is a template provided by HR. We have specific categories that are rated 1-5 by the employee and their manager. There is also a field for comments, etc. This would be where you would provide your justification for your numerical rating. Finally at the end there are sections titled "Opportunities for Improvement", "Accomplishements" and "Goals" along with a "Comments" field.

I have a cleaned up version in Word that I can send to you if you think it would be helpful. Let me know, thanks!

Re: Annual Self-Review
What we use in Higher Ed is a form employee self review worksheet. There are a couple of ideas that you may want to incorporate in your summary. 1. List what you believe were the key elements of your job during the past year. 2. List special contributions you feel you have mde to your dept. during the last appraisal period. 3. List any notable obstacles you encountered in accomplishing your job responsibilities during the appraisal period. 4. What areas do you need (want) to develop in the next year? What are your plans to achieve these goals? 5. What can your supervisor do to better assist you in your position? 6. Add any additional information which you wish to have considered in your appraisal. Hope this will give you some ideas to expand upon from your current format. Good luck!

Re: Annual Self-Review
If your company has "values", it's always a good idea to focus your self review on specific ways you demonstrate those values through your work.

For example, if one of the company values is "financial strength" you could give several examples of how you have saved your department (i.e, the company) money by revamping the office supply ordering system, or developing a tracking method for office supplies.

Anything that shows how you are contributing to the overall mission and vision of the company should be included on your self-review. Most on-line documents generated in HR will usually follow that format, then as mentioned above, break down each value into separate areas of job performance.

Re: Annual Self-Review
Thank you all for your input. We are a small company of 8, so we don't have an official HR dept. I have been doing these SRs for the past 8 years, sort of nilly willy and don't feel they represent what I accomplish or do on a day to day basis. I guess one of my goals for '09 is to work on the Self Review.

Re: Annual Self-Review
One quick comment here regarding sharing your HR eval document. Please confirm with your HR department that you are authorized to share the document outside the company. Any documents generated through HR (even if they are using a software program for evals) could be considered company property. I would hate to see anyone get fired for sharing corporate docs!

Just check to make sure you have permission to share the template.
