I think number 5 is especially important. After a negative review, keep records of what you are doing to improve the situation and SHARE that information with your manager on a very regular basis. Show your concern and your determination to make the situation better. Also, if it's not a part of the review, create an Action Plan for yourself - outline specific ways in which you plan to address the issues discussed and prevent another negative review from happening. Share that with your manager as well. And follow up often to show that you're sticking with it.
