Another answer to Number 5 is: None of the above. Many companies have an ethics policy in place that states employees are not to purchase meals, drinks, gifts, etc. for supervisors, managers or anyone whose position is of a higher rank. This was put into place to avoid accusations of bribery or favortism.

Excellent point, Phyllis! At my previous company, that was certainly the way it was. Once, we went to New York and there were about fifteen of us at dinner. The highest ranking person had to put the meal on his PERSONAL credit card and then get reimbursement. He was totally annoyed and we just all laughed and said, "That's what you get for climbing the corporate ladder!"
