My impression is that Board minute formats vary based on the governance model adopted by the Board (and the preferences of your exec). Our Board uses the Carver model of governance and I've attached a samples of the agenda/minutes format we use (as a nonprofit, our records are open to the public so I can post them).

It may also be a good idea to look up your employer's previous Board minutes and ask your boss what the advantages and disadvantages of that format are.
