This is great, but I would ask one favor, if possible. Could you please either unprotect the document, or give us the password to unprotect it, so we could modify it for our own use? Everyone doesn't have the same requirements for Office Supplies.

Ummmm...thanks. This is definitely customizable now, but it is no longer a "form" with blanks to fill in. I thought that was the improvement over the previous version. However, this will make a good starting point to create our own form - I can always add the form fields back in on my own, and protect it. I think I had expected that you would just remove the password, leaving the form fields on it. It could be customized that way, then re-protected as a form with fields to fill in, just leaving off the password. I do that with a lot of my forms, and unless it is something that I really don't want anyone to change in any way, I leave the password field blank when I protect it. I understand that if you protect a form like that and forget the password, it's pretty hard to go back and change it. Thanks again for the quick response.
