Emergency Contact Information (PDF Form)
Description:
Let's face it: emergencies happen. And when they happen at the workplace, it can be a real challenge. Maintaining an up-to-date Emergency Contact List for every employee is extremely important. Have everyone review their information on an annual basis and keep both paper and electronic copies.Though emergencies are never fun, having this information can make the process a lot easier to handle.

*nods* That's why I recommend people with severe issues wear a medic alert bracelet. And I'm not saying that people shouldn't share information with coworkers about severe health issues and what to do just in case. I used to be a paramedic, so I know how valuable that information is in an emergency. I'm just saying that employers shouldn't request the information. It can open up all kinds of cans of worms.
