You can put the whole name in one field or you can break down the name into two separate fields: first name and surname. When it comes to the address field however, you have more variables in play. You can do anything from placing the address in 1 field to breaking it down to the smallest elements: street number, street name, apartment number, city, zip code and state. What you choose will be based on your needs and how the organization will use the information. If you will be sending out sales flyers by where your clients live, then sorting by state or zip may be a realistic need.
